CWP Application for Professionals

Certified Wellness Professional (CWP) application must be submitted through an online form and applications are reviewed once all materials and full payment are received. Applicants will then be notified of their acceptance status via e-mail within 4-5 weeks. Approved applicants will receive an official certificate in the mail.


Applications submitted before August 1, 2017

To be eligible to submit a CWP application, individuals must:

      hold a minimum of a bachelor's degree, and

      have five years of professional wellness experience


The CWCE uses the CWP Evaluation Rubric to determine if the applicant has met the minimum required standards to hold the CWP credential.


Applications submitted after July 31, 2017

To be eligible to submit a CWP application, you must meet one of the following educational and work experience pairings at the time of application submission:




All CWP applications must include the following:


1. OFFICIAL transcripts documenting your academic preparation, degree(s) and educational background. Transcripts from each degree completed are required. If you received your highest degree from an institution outside the United States, please apply for a WES credential evaluation (additional fee through WES) and submit an official credential evaluation report in lieu of transcripts. Send official transcripts or official credential evaluation report to:

2. Your résumé/curriculum vitae providing evidence of full-time, post-degree work experience in the wellness/health promotion field where wellness/health promotion is a direct responsibility in your job description. Include job descriptions and names (along with telephone numbers and emails) of references who can speak to your wellness leadership. Volunteer, leadership, and continuing education experiences should also be included in a separate category within the résumé /CV. Upload to the online application form.

3. Application Fee of $365* ($50 non-refundable) payable to the National Wellness Institute. Payment may be made online after submitting the online application form. If “pay by check” is selected, application will not be reviewed until payment is received.

Upon approval, the term of certification is five years and automatically includes a five-year membership in the National Wellness Institute (NWI) at no additional charge (currently, a $675 value). If your application is not approved, you will receive a refund of $315. 

Applications submitted before August 1, 2017, only:
4. Personal statement of qualifications (750 to 1,000 words) providing specific examples of:
how your academic background and work experience demonstrate your competency in applying knowledge and skills from the core content areas (see CWP Evaluation Rubric).
how you integrate multiple dimensions of wellness into your approach to promoting wellness. Provide TWO specific examples of how you have used a whole-person, multi-dimensional approach to working with individuals, families, and/or communities. 
ways have you continued to enhance your wellness knowledge (provide documentation).

Applications submitted after July 31, 2017, only:
4. Wellness Promotion Competencies form (Download fillable PDF here). Draw selectively from your academic preparation, work experience, and continuing education to provide meaningful evidence that collectively you demonstrate proficiency in the required competency domains.

NOTE: The form is a fillable PDF and should be downloaded and opened with Adobe Reader (install for free here: Opening the PDF in your browser window may not allow you to fill it out and save it properly. To save the completed form, use the button at the bottom of the form (or choose File > Save As from the Adobe Reader menu) and rename the file. Then, upload it to the online application form after July 31, 2017.

All fees and terms subject to change at the discretion of the National Wellness Institute and the Council on Wellness Certification Excellence. 

If you plan to apply using the Competencies for Wellness Promotion form, please do not submit your application until August 1, 2017.  




After submitting the form you will be redirected to a payment screen to submit your application fee. Applications will be reviewed once all materials, including official transcripts and full application fee, are received. Applicants will be notified of acceptance status via e-mail 4-5 weeks after all materials are received.
